Our Approach to Insurance and Billing
We believe in being direct about insurance and billing. T1D Hub works with a select group of insurance plans rather than every plan available. This approach allows us to spend more time with each patient and keep our practice focused on thoughtful, relationship-based primary care for patients in Mishawaka, South Bend, and the surrounding communities.
Because insurance networks and plan details change often, the most reliable way to confirm whether we work with your plan is to contact our office before scheduling. We’ll review your plan with you and explain what to expect. If we don’t work with your plan, we’ll tell you plainly.
What to Bring to Your Appointment
Please bring the following to each visit:
- Your current insurance card, along with any secondary coverage cards
- A photo ID
- A form of payment for any copay your plan requires
If your insurance has changed since your last visit, let our team know before your appointment.
Understanding Your Insurance Benefits and Costs
Your insurance plan determines what’s covered and how much you’ll owe. Depending on your plan, you may be responsible for:
Copays
A set amount you pay for a visit or service.
Deductibles
The amount you pay each year before your plan begins to share costs.
Coinsurance
A percentage of the cost you pay after meeting your deductible.
Non-Covered Services

After your visit, we submit claims to plans we work with on your behalf. Your insurance company then processes the claim and sends you an Explanation of Benefits (EOB) that shows what your plan covered. An EOB is not a bill. Any remaining balance will appear on a statement from our office.
Paying Your Bill
Dr. Huber designed T1D Hub around modern technology, and we aim to make paying your bill as simple as the rest of your care, with as little paper as possible. Does T1D Hub accept my insurance?

I’m a family member of someone with type 1 diabetes. Can I become a patient?
T1D Hub provides primary care for individuals living with type 1 diabetes and their families. Because each family member’s coverage depends on their own insurance plan, we recommend verifying coverage for each person before scheduling.
Will my insurance cover diabetes technology like CGMs and insulin pumps?
Coverage for diabetes devices and supplies varies by plan, and many plans require prior authorization. Dr. Huber has extensive experience with insulin pumps and continuous glucose monitors, and our team can help you understand what your plan requires.
What’s the difference between an EOB and a bill?
An Explanation of Benefits comes from your insurance company and explains how your plan processed a claim. A bill, or statement, comes from our office and shows any balance you owe after your plan has paid its portion.
What should I do if my insurance changes?
Let us know as soon as possible, ideally before your next appointment. We’ll update your records and confirm your new coverage, so there are no surprises.
